Team Event Form
If your team is doing a Team Fundraiser, Team Event, Exhibition Game or Tournament, please fill out this online form. Teams that will be participating in Exhibition games, must ensure that all players are ASA insured and are eligible to play.
Please note that for teams traveling outside of Alberta, they MUST fill out the ASA Travel Permit and email it to our office at office@spdsa.net
Season Player Cards
As of Friday, November 14th, all coaches have been provided with player and staff cards. Player Cards will be required for u13-Adult league games starting November 15th, 2025. Team Official Cards will be required for u11-Adult League starting November 15th, 2025 as well. Players and Coaches without cards will NOT be allowed to participate. Please contact office@spdsa.net if you have any concerns.
Game Sheets
Please fill these game sheets out prior to your kick-off time. Game sheets are submitted to the referee to complete during the game. Referees will submit completed game sheets to the Phoenix FC Office.
Team Managers or the designated Time-Tracker can download the team hour-tracking sheet. Please submit this to office@spdsa.net at the end of the season, or once the volunteers on your team have completed their minimum required hours.
Team-Volunteer Hour Tracking
Please submit team-based volunteer hours once the volunteers on your team have completed their minimum required hours.
Our office will not track individual hours for team-based roles.
