If your team is interested in attending Provincials for the upcoming season, Phoenix FC will need the following application filled out.

  • Recreational (Tier 4) Teams - The highest placed team that declares for Provincials will represent Sherwood Park at Tier IV Youth Provincials*.

  • Competitive (Tier 1-3) Teams - All Tier 1-3 teams must also fill out the EMSA form. Phoenix FC does not have a automatic birth, All Tier 1-3 teams must earn their sport through the EMSA League.

  • Recreational (Tier 4) Adult Teams - Phoenix FC does not send teams through provincials from our in-house adult leagues.

*The SPDSA will have the final decision on a team's eligibility for Provincial Competition.
